April Month Request

Guys,here are few things to be highlighted.

1. All the patient's cards must be signed and chopped in every zone.
2.All the death cases (DID), make sure the BDM form must be signed and chopped
3.Make sure patient's card not left empty unless CNA. And all the CNA cases must be given to PPP
   incharge.
4.We are trying to make sure the numbers of MO per shift is not less than 4 in order everyone can          enjoy working in our dept . So co-operation needed from all of you,no changes or request after the      roster is published.
5. ALL the EL are currently be monitored by Dr Teo. Unnessesary ELs are not allowed.

    9th - Night
    10th - Post Night
    11th - Week off
    12th - Night

    23rd - Night
    24th - Post Night
    25th - HK
    26, 27 and 28 - Cuti Ganti for Thaipusam and CNY
    29th of April til 7th May - on leave

      One more request,missed out!

      Need night off on saturday (11/4).If possible week off,if not
      plz plz plz schedule me night shift on 10/4 and pm shift on 12/4.
      Have a wedding to attend in Ipoh,need to be there by noon on 11/4.Thank you very much!

    Hi dr Sasi and dr geeta, hopefully it's not too late...
    Initially requested for post night off or weekoff on 4/4 for tomb sweeping....after reconfirmed with aunt, it's actually on the 5/4, so would like to change the night off/weekoff to 5th. Thanks!
