January Request

Dear friends, HAPPY NEW YEAR

2016 is just around the corner. Hope everyone start targetting for new year resolutions.
Starting from January we will implement the compulsory CME attendence in order to achieve the required hours.
I expect once the roster is published,the leave form should be summitted ASAP.
The C & P forms will be ready by midweek of December,so start anticipating. TNX

      sorry for the trouble, but there are some changes in my january request.

      30/12-3/1 : anything. pls ignore previous request.
      14/1: postnight
      15/1: week off
      16/1 : rph new year (best friend's pre wedding rehearsal)
      17/1 :AL (best friend's wedding in kl)
      18/1 :AL (travelling back to penang)
      23/1: week off
      24/1 : PH thaipusam

    i will be on leave from 10th Jan to 25th january
